My knowledge of the fact and date of such Birth is based on the following Circumstances:

I am the applicant and I have always celebrated my birthday on the 2nd day of December and I have a certificate of Baptism dated July 12th, 1952 indicating that I was baptized on January 11th, 1925, and that my birthdate was December 2, 1924.
