My knowledge of the fact and date of such Birth is based on the following Circumstances:
Certification from church records of Chalmers United Church, Dominion. Record from the office of the superintendent of Schools, Glace Bay Personal knowledge as mother of the child. The child was apparently incorrectly recorded as Charles Henry instead of Chesley Henry
