My knowledge of the fact and date of such Birth is based on the following Circumstances:

My mother and father always stated that I was born as stated above. I always presumed that my birth was registered until I applied for a certificate. I enclose a baptism certificate showing my baptism occurred on the 13th day of July, 1924. My mother and father are now predeceased.
