That Helen RoseGouthro Whose Birth is sought to be registered was

born at Glace Bay Cape Breton County on April 28, 1923

My knowledge of the fact and date of such Birth is based on the following Circumstances:

I am the mother of the child. The Baptismal register was destroyed by fire along with St. John's Catholic Church in New Aberdeen where such record had been kept. We have celebrated Helen's birthday on the 28th day of April for 52 years and there is absolutely no doubt in my mind as to the date or for that matter, the year of birth. The certificate of the Superintendent of Schools for Town of Glace Bay is appended as an exhibit to this affidavit and I confirm that the information set out in such certificate is true
