My knowledge of the fact and date of such Birth is based on the following Circumstances:

I am his mother and distinctly remember the circumstance and am sure the above date of birth is correct. I have already forwarded various proofs including copy of Church record of marriage, copy of school record, as furnished by supervisor of Amherst Schools. Also Affidavit of Nurse in attendance at birth. School record showing month of birth in error as February - the correct month is March.
