My knowledge of the fact and date of such Birth is based on the following Circumstances:

I was with the doctor at the birth. Also (my husband was deceased) and I had the child baptized and it is on the church record, the copy enclosed, signed and written by our present Minister from the record. the mother of the child died a few days after the birth so I am Foster mother as well as grandmother.
