My knowledge of the fact and date of such Birth is based on the following Circumstances:

This child was admitted to St. Marys Home, now known as Mercy Hospital, Sydney on December 19, 1923. Said child being brought to this Institution by the mother, Mary Morrison. I have seen the file of the Institution containing information as to this childs name, date and place of birth and the name of the childs mother. Actual personal knowledge based on certificate of Baptism attached hereto.
