My knowledge of the fact and date of such Birth is based on the following Circumstances:

I had the Pastor of Holy Redeemer Church provide me with a Certificate of Baptism on July 10th, 1948 and it confirmed according to church records made at the time that I was born on March 10th, 1921 and bpatised on March 13th, 1921. Attached hereto as Schedule "A" is a copy of the Certificate of Baptism.
