My knowledge of the fact and date of such Birth is based on the following Circumstances:

On my Baptismal Certificate and my School Register and also on the fact that I have always regarded my birthday as the 16th of July 1921 and it was also regarded by my father and mother and the other members of my family all of whom are registered and could furnish affidavits of the date of my birth if necessary save my mother and one younger brother who are dead.
